Requests for subsequent Income Withholding Orders for Child Support will be processed on the Revised Order/Notice to Withhold Income for Child Support form.
The revised Order form, instructions to complete the order form and an Information Sheet is attached. The attached order fonn is the actual order that will be presented to the Judge for signature and will be served on the employer, for this reason, it is important that you PRINT LEGIBLY.
The clerk’s office cannot help you complete the order form. Using the instructions provided, complete the entire form with the exception of those portions titled ORDER INFORMATION, REMITTANCE INFORMATION and the lower left hand side where the Judge’s signature is required.
The fee to cover the costs of service of the subsequent Income Withholding Order must be paid at the time the request is submitted. If the employer is to be served by certified mail, i.e. out of state address or post office box address, the fee will be $8.00. If the employer is to be served by personal service, i.e. address is within the state of Alabama and you provide the actual street address, the fee will be $10.00.
